There are two ways to find your drug within the formulary: Medical Condition The formulary begins on page 8. The drugs in this formulary are grouped into categories depending on the type of medical conditions that they are used to treat. For example, drugs used to treat a heart condition are listed under the category, Cardiovascular. If you know what your drug is used for, look for the category name in the list that begins 8. Then look under the category name for your drug. Alphabetical Listing If you are not sure what category to look under, you should look for your drug in the Index that begins on page 17. The Index provides an alphabetical list of all of the drugs included in this document. Both brand-name drugs and generic drugs are listed in the Index. Look in the Index and find your drug. Next to your drug, you will see the page number where you can find coverage information. Turn to the page listed in the Index and find the name of your drug in the first column of the list. What are generic drugs? Envision Rx Plus covers both brand-name drugs and generic drugs. A generic drug has the same active-ingredient as the brand name drug. Generic drugs usually cost less than brand name drugs and are approved by the Food and Drug Administration FDA. General Practice mental health care GPmhc ; Program This program is open to GPs registered with HIC for the Better Outcomes in Mental Health Care Initiative. Since it began in September last year we have received 38 referrals and 2 re-referrals. Over 100 occasions of service have been provided to these patients. However, in the last month the program has received fewer than 10 referrals. The 2 psychologists contracted to the Division have a capacity to undertake 15 consultations each per fortnight so keep those referrals flowing. If you are interested in accessing this program for your patients but are not yet registered, please contact either myself or Tanja McLeish at the Division. The Division has also been successful in submitting a poster presentation on the program to the 9th Rural Mental Health Conference to be held in Armidale March 16-18, 2004. The poster highlights the lack of sp ec ial is t m tal he al th clinicians working in rural areas and the work undertaken by.

Mr PF, a 35-year old office worker, presented to his optometrist in severe pain with a watery red right eye. One week ago, his right eye had become uncomfortable whilst on holidays in Singapore. He had been swimming in his CLs. Mr PF ceased CL wear, but his discomfort progressed to severe pain and the eye became red and his vision blurred. Mr PF had a 10-year history of soft CL wear for moderate myopia. He wore his CLs four to five days a week for 10 hours a day. After wear, he cleaned his lens with a chlorine disinfection system and disposed of them every month. His CL storage case was over one year old. Twice before he a red eye from CL wear; he could not recall which eye was affected or his treatment. His general health was good apart from knee pain due to an old football injury. He had no regular medications, took analgesics as required and had no allergies. Socially, he drank two to three pints every other day and did not smoke. There was no significant family ophthalmic history.

THE FACULTY OF BUSINESS AND ECONOMICS made steady progress in 2002. Firstly, the number of refereed journal articles increased 35% from 2001. This is particularly pleasing as it is key performance-based research funding PBRF ; assessment criterion. Secondly, with PBRF imminent, a research taskforce was convened in 2002 to identify ways of raising the school's level of research activity and output. This taskforce made several recommendations in September. Thirdly, Neville Mercer was appointed as Research Manager. His role is to provide information on external research funding opportunities and help prepare grant applications, and there was an increase in major applications from the faculty to external funding organisations. Notable research successes in 2002 included: Dr Judy Motion received a $2.5million FoRST grant to examine the social and cultural impacts of biotechnology in New Zealand, with Victoria, Waikato, AUT and HortResearch. An Industry New Zealand research grant of $400, 000 over two years went to Associate-Professor Shantha Liyanage to develop innovation and entrepreneurship skills in biotechnology. The main programme will focus on high school and intermediate students and their teachers, with smaller programmes providing, for instance, solubilizing.

Thyroid nodule shape suggests malignancy. Eur J Endocrinol 2006; 155: 27-31. SUMMARY Background Thyroid nodules are common, but only a few are carcinomas. This study was done to determine whether benign thyroid nodules can be distinguished from carcinomas by the shape of the nodule or other characteristics, as determined by ultrasonography. Methods The ultrasound and cytologic characteristics of 7455 thyroid nodules in 5198 patients seen between 1991 and 2004 were retrospectively evaluated. Among the nodules, 2865 38 percent ; were 1 cm in diameter. The ultrasound scans were evaluated by two investigators, who recorded the following information for each nodule: anteroposterior and transverse A T ; diameter, degree of echogenicity, presence of calcification, characteristics of nodule margin blurred or well-defined ; , and vascularity. The nodules were biopsied using 25-gauge needles, and the results were reported as benign, suspicious including follicular and Hurthle-cell tumor and suspicious for papillary carcinoma ; , malignant, or inadequate 6 large clusters of follicular cells ; . All patients who had biopsies reported as suspicious or malignant underwent surgery. Nodules that not been biopsied that were carcinomas were not included in the analysis. Results Among the 7455 nodules, 6135 biopsies 82 percent ; were adequate and 1320 18 percent ; were inadequate; the respective numbers of patients were 4495 3118 women, 1377 men ; and 703. Nodules for which the biopsy was inadequate were excluded. Multiple nodules were successfully biopsied in 1351 patients 30 percent ; , of whom 1118 had 2 nodules and 233 had 3 nodules. Among the 6135 nodules that were successfully biopsied, the mean longest dimension was 16 mm range, 6 to 100.

The concern of Dr. Roth with the treatment of this patient was that Dr. Vaidyanathan provided contraceptive counselling while the patient was on the operating table. A more reasonable approach would have been to set up an office appointment to discuss contraceptive options. Nonetheless, in this case, Dr. Roth opined that the management of this patient, while not entirely acceptable, met the standard of care.
Olorectal cancer CRC ; is a major health concern, with more than 944, 000 cases and 492, 000 cancerrelated deaths reported worldwide in 2000.1 In the United States, CRC affects nearly 150, 000 people each year, with as many as 56, 730 deaths anticipated in 2004.2 These statistics are sobering, considering that in most instances CRC should be preventable by wellestablished screening and surgical techniques. Nevertheless, CRC death rates remain unacceptably high, calling for better compliance with screening guidelines and the identification of complementary strategies to reduce the public health burden of this disease. One promising strategy, cancer chemoprevention, is based on understanding the molecular determinants of cancer. Cancer chemoprevention strives to block, reverse, or delay carcinogenesis before the development of invasive disease by targeting key molecular derangements using pharmacological or nutritional agents.3 Chemopreventive interventions may be applied at any time during carcinogenesis, from the initial molecular defect through the accumulated molecular, cellular, and histopathologic aberrations that characterize disease progression before an invasive--and potentially metastatic--stage. SLE is a disease which may involve different organ systems to varying degrees. The effect of the disease on the various organs may lead to painful conditions that may be acute or chronic in nature. Chronic pain is more difficult to treat than acute pain for two reasons. First, by definition, the pain is persistent and, presumably, has failed to resolve itself spontaneously or to respond to previous treatments. Secondly the impact and duration of the pain ultimately creates altered behavior in the patient. The objectives of this lecture will be to review the currently used terminology of pain, the neurophysiology of pain the more commonly experienced pain disorders associated with lupus, and pain treatments, We will also discuss the importance of a multidisciplinary approach to chronic pain management. The International Association for the Study of Pain IASO ; has defined pain as "an unpleasant sensory and emotional experience associated with actual or potential tissue damage, or described in terms of such damage". 1 ; This definition may be surprising to many people as it recognized pain not as a sensation but as an experience. We recognize in science that virtually all humans are endowed with five senses: sight, smell, hearing, touch and taste. To recognize pain as an experience rather than a sensation, one must first recognize that sensations neuroanatomically have discrete pathways with specific receptors to allow detection and measurement of a stimulus. In contrast, an experience incorporates sensory components with important personality and environmental influences. The sensory component of pain is described as nociception. Nociception is the physiological response to tissue injury. Nociception is the progenitor to the experience of pain, which in turn creates suffering. Nociceptive pain and suffering are internal to the organism and cannot be measured directly. However, they do create recognizable impacts on behavior, which can be identified, described and subsequently measured. 2 ; The medical treatment of pain is facilitated by a basic understanding of the neuroanatomy and physiology of pain processing. The nociceptors are pain sensitive neurons that respond to noxious mechanical, chemical, and thermal forces. They are located in the skin, vessels, muscles, fascia, joints, and viscera. They are defined as A delta and C fibers. These nociceptors make up the prima!

This medication might cause dizziness or light-headedness, especially if patient rises suddenly from a recumbent or seated position. Guidelines are based on experience derived from treating Caucasian patients, who are more likely to develop photoaging earlier than darker-skinned individuals. Recent experiences in those with Asian skin have led to similar guidelines. Typically these patients are in otherwise good health with no known contact allergies to ALA, and no photosensitivity disorders. In particular, patients who have any form of porphyria, connective tissue disease, solar urticaria or polymorphous light eruption should be excluded from treatment, for fear of symptom exacerbation. It is good practice to also withhold PDT from patients taking known systemic photosensitizing medications. Examples of these are listed in Box 8.4. Lastly, as the administration of ALA causes photosensitivity.

Company has effluent treatment plant at its bulk pharmaceuticals manufacturing facility located at Bollaram Industrial Development Area. Company also started a new trend in the Indian pharmaceutical sector by installing a "satellite" discovery research laboratory in the United States. In April 2001 Company got listing in the New York Stock Exchange NYSE ; . Company merged "Zenovus Biotech Ltd.", a wholly owned subsidiary to enhance its strength into biotech. Company having over 950 scientists working across the globe, around the clock, the company continues its relentless march forward to discover and deliver a breakthrough medicine to address an unmet medical need and make a difference to people's lives worldwide. Products & services-- Company manufactures and markets Active Pharma Ingredients, Finished Dosages and Biologics in over 100 countries worldwide, in addition to having a very promising Drug Discovery Pipeline. When Dr. Reddy's started its first big move in 1986 from manufacturing and marketing bulk actives to the domestic Indian ; market to manufacturing and exporting difficult-tomanufacture bulk actives such as Methyldopa to highly regulated overseas markets, it had to not only overcome regulatory and legal hurdles but also battle deeply entrenched mind-set issues of Indian Pharma being seen as producers of 'cheap' and therefore `low quality' pharmaceuticals. Today, the Indian pharma industry, in stark contrast, is known globally for its proven high qualitylow cost advantage in delivering safe and effective pharmaceuticals. This transition, a tough and often-perilous one, was made possible thanks to the pioneering efforts of companies such as Dr. Reddy's Laboratories. Company's product portfolio spans an impressive mix of Active Pharmaceutical Ingredients, Generics and Branded Formulations as well as Biologics. In the API Segment Company manufactures Anti viral, Anti Hypertensive, muscle relaxant, Anti bacterial. 71 ; PENTECH PHARMACEUTICALS, INC.
