| T wave morphology: Moss, et al first reported a T wave pattern characteristic for each genotype.25 Zhang, et al further described patterns characteristic for each genotype, reporting four for LQT1, four for LQT2 and two for LQT3.26 These T patterns can be helpful for predicting the correct genotype in families, and can be of assistance in the diagnosis of LQTS in cases of borderline QT duration. Is LQTS the cause of the QT prolongation? It is important to note that there are a number of causes of QT prolongation other than inherited LQTS. Included are electrolyte disturbance, use of QT prolonging medications, mitral valve prolapse, diabetic autonomic neuropathy and cardiomyopathies. These conditions must be excluded when evaluating a patient who has a prolonged QT interval before a confident diagnosis of the Long QT syndrome can be made. Current evidence suggests that in the absence of these confounding factors, a QTc of 480 msec in females and 470 msec in males allow the diagnosis of LQTS. QTcs of 410 msec make LQTS quite unlikely. Values between 410 and 460 msec are ambiguous and further testing must be performed to clarify the status of these patients. That further testing includes additional ECGs, ambulatory ECGs and exercise ECGs. Exercise ECGs seem to be the most definitive. Genetic testing is helpful when available, see below. Clinical Course and Pathophysiology General: Syncope is the predominant symptom, and patients may have one to several hundred episodes. For the first time, laboratory scientists have demonstrated a model that may explain how alcohol stimulates tumor growth. A study reported in the January 2005 Cancer suggests that alcohol fuels the production of a growth factor that stimulates blood vessel development in tumors. According to the American Cancer Society, for almost a hundred years, mounting epidemiological evidence has linked alcohol use to an increased risk of cancers of the stomach, esophagus, liver, breast, and colon. Jian-Wei Gu, MD, from the University of Mississippi Medical Center and colleagues investigated the possible mechanism between ethanol-induced blood vessel growth and VEGF in an animal study. During their laboratory research, embryos exposed to ethanol had more than eight times the level of cancer cell invasion of blood vessels compared to the control group exposed to a saline solution. The embryos exposed to ethanol experienced increases in tumor size, tumor blood vessel density, cancer cell infiltration of blood vessels, and VEGF levels. METABOLIC SYNDROME AND CLOZAPINE In a study that compared 93 current patients receiving clozapine to over 2, 700 matched controls, the prevalence of patients developing metabolic syndrome was well over 50% of the active population.7 The comparators were drawn from the most recent version of the National Health and Nutrition Examination Survey NHANES ; . The rate of development of metabolic syndrome was 20.7% in the matched group compared to 53.8% of the clozapine group. It has long been known that clozapine and some of the other, newer atypical antipsychotic drugs have been implicated in the development of metabolic syndrome, including de novo diabetes in great numbers of patients. This is the first study to actually look prospectively at the development of this potentially severe adverse effect. INCREASED ADRS TO ANTIMICROBIALS AND ANTICONVULSANTS IN AIDS PATIENTS An excellent review of the problems of increased adverse drug reactions in patients infected with HIV was recently published.8 This review looked at well over 100 literature citations and discusses the possible mechanisms involved that cause HIV-infected individuals to have greatly increased morbidity and mortality when they are treated with sulfa drugs, antimycobacterial agents, and anticonvulsant medications. The severity of the adverse effects certainly leads to poorer drug adherence and some and zidovudine.
A 29-yr-old woman with a 12-yr history of bipolar disorder with psychotic features was admitted through the emergency room with increasing hallucinations, disorientation and agitation of four weeks' duration. Her medications on admission included lithium carbonate, divalproex sodium, and clozapine. Her past medical history was significant for three previous hospitalizations due to acute psychosis, which responded to various antipsychotic medications. She had never undergone general anaesthesia, and had no personal or family history of malignant hyperthermia or other anaesthetic-related problems. On physical examination, the patient weighed 49 kg, had an arterial blood pressure BP ; of 110 60 mmHg, sinus tachycardia at 100 beats min~', and was afebrile at 98 E She was anxious, yet cooperative, with increased psychomotor activity. Thought content revealed delusions of persecution and auditory hallucinations. Cognitive testing revealed the patient to be orientated, but with decreased attention span. The rest of the mental and physical examinations were essentially normal. Serum concentration of electrolytes, liver function tests, thyroid function tests, serum glucose and complete blood count were all within normal limits. Her chest radiograph and electrocardiogram ECG ; were unremarkable.

The regulation of insulin secretion from pancreatic -cells is a complex physiological process controlled by a number of interacting signaling components, including acetylcholine 13 ; . Released from the vagus nerve, acetylcholine stimulates insulin secretion through the M3 muscarinic receptor subtype coupled to phospholipase-C activation 14, 15, 22 ; . Under our experimental conditions, 10 mol l carbachol produced a biphasic insulin secretory response in the presence of physiological glucose levels 7 mmol l ; , with increases in second-phase release rates of three- to fivefold above prestimulatory rates. The addition of 10 mol l atropine, olanzapine, or clozapinw completely blocked carbachol-stimulated insulin secretion, whereas ziprasidone, risperidone, or haloperidol had no effect. These concentrations are near the therapeutic free plasma levels of olanzapine and clozapine in patients 23, 24 ; , although tissue levels may be higher 25 ; .
