Background: In Sweden 40 000 women annually receive information that their Pap-smear test shows dysplasia and about 400 women are diagnosed with invasive cervical cancer. The aim of this study was to evaluate women from the County of Vrmland who had two consecutive Pap smears with mild dysplasia by describing their experience of receiving information about the results and how examinations, treatment and follow-up had affected them 1-3 ; . Methods: A questionnaire was sent in 1999 to 329 women who according to the Department of Clinical Pathology, Karlstad had two consecutive atypical Pap smears during 1993 and who as a result of this finding should have undergone colposcopy and biopsy according to an agreed general program. Results: Two hundred forty-two women 74% ; completed the questionnaire. The finding of mild dysplasia was obtained as a screening test in 95%. Two hundred and thirty-two women 96% ; reported that they had participated in the follow-up program. One hundred and eightyfour women 76% ; experienced follow-up in a positive way. Seventy-two percent considered they understood the meaning and consequences of having a mild dysplasia. Nevertheless feelings of worry and anxiety affected 59%. For 30% it affected every day life between being informed of the result of the first Pap-smear test and subsequent further investigation. Twenty women 8% ; reported a remaining negative influence on sexuality and of sexual intercourse as a consequence of the treatment of mild dysplasia. This was correlated to less satisfaction with follow-up, and a negative influence on self-esteem 4-10 ; . Conclusion: Women felt vulnerable when the mild dysplasia was discovered, but this did not influence willingness to participate in follow-up. The discovery created unnecessary worry and a negative experience that may be solved by a better-developed educational program at the time of screening. However, tcas are associated with a complex side effect profile and a slow time of onset action, therefore they are more difficult medications for practitioners to use and for patients to tolerate.
Occurred more often in women than in men. In addition, the response rate was greater in women who became hypotensive than in women who did not become hypotensive P .039 ; . These findings provide further support for increasing ILl a dose intensity to increase response rates. The induction of a procoagulant state by IL-la may enhance its antitumor effects in the setting of hypotension. In vitro, IL-la activates endothelial cells to synthesize prostaglandin I2, prostaglandin E2, and platelet-activating factor, increases endothelial cell adhesiveness that increases leukocyte adherence and increases plasminogen activator inhibitor levels 22 ; and in vivo, decreases protein C levels 11 ; . These activities produce vascular congestion and promote clot formation that, in combination with hypotension, may account for the tumor necrosis induced by IL-la as observed in some animal tumor models 27 ; . One patient with pulmonary parenchymal metastases developed recurrent episodes of hemoptysis, possibly a clinical correlate of tumor necrosis, during treatment in association with disease regression. Indometthacin augments the antitumor activity of IL-la in animals and, on that basis, was included in this study. Several studies 28, 29 ; suggest that prostaglandin synthesis inhibitors have favorable immunomodulatory activity in the treatment of cancer, although prospective clinical studies 2829 ; of these agents have failed to improve response rates 30, 31 ; . Miller et al. 50 ; randomly assigned patients with metastatic melanoma to treatment with interferon alfa alone or in combination with indomethacin. They observed a response rate of 13% in both arms of the study. Creagan et al. 31 ; randomly assigned patients with renal cell cancer to treatment with interferon alfa alone or in combination with aspirin; an earlier nonrandomized trial showed a dramatic improvement in response rate with the aspirin combination 25 ; that was not confirmed in the prospective randomized trial 31 ; . The importance of indomethacin for response seen in this trial is unclear; however, on the basis of the other studies, it is anticipated to be minimal. This study shows that the combination of IL-la and indomethacin has minimal antitumor activity in the treatment of.

In the 1960s, Goldhaber and colleagues were studying mechanisms of localized bone resorption in order to better understand specific factors that might be important in the bone loss of periodontal disease Goldhaber et al., 1964 ; . These investigators reported that culture media from human gingival tissue fragments produced a bone resorption-stimulating factor that could stimulate bone resorption of mouse calvaria in tissue culture. Media from several gingival tissue cell lines, including epithelial cells and fibroblasts, also induced bone resorption in tissue culture Goldhaber, 1971; Goldhaber et al, 1973 ; . At about this same time, Klein and Raisz reported 1970 ; , in their landmark paper, that prostaglandins were potent stimulators of bone resorption in tissue culture. It was also at this same time that John Vane 1971 ; reported that aspirin and aspirin-like drugs the nonsteroidal antiinflammatory drugs or NSAIDs ; inhibited the production of prostaglandins by inhibiting the enzyme cyclooxygenase. Thus, given the above series of discoveries, Goldhaber and coworkers examined whether prostaglandins might be responsible in part for the gingival-tissue extract mediated bone resorption they were studying by adding indomethacin to the culture media, as an inhibitor of cyclooxygenase. The prostaglandin production blocked by indomethacin decreased the bone resorption in tissue culture by up to 50% Goldhaber et al., 1973 ; . This series of exciting findings suggested that some factor in gingival tissue, and in specific cells from gingival tissues, had the ability to stimulate bone resorption by apparently stimulating the bone itself to produce the resorption. It was not clear at the time what this finding might mean in vivo in periodontal disease, but it did link gingival tissue with bone resorption. Second, it appeared that one of the primary pathways whereby gingival tissue influenced bone resorption in tissue culture was through prostaglandin production by bone. Thus, prostaglandins could be significant mediators of the bone resorption of periodontitis. Finally, if indomethacin blocked the bone resorption of gingival fragment media-induced resorption. it was hypothesized that indomethacin and other inhibitors of prostaglandin production NSAIDs ; might have a role in slowing or block.
Etodolac is a new non-steroidal anti-inflammatory drug. Indications: Long term treatment of rheumatoid arthritis and osteoarthritis. Mechanism of Action: A prostaglandin synthetase inhibitor with some selectivity for cyclooxygenase 2 enzymes. Pharmacokinetics: Oral bioavailability is 80%. Inactivated primarily by liver metabolism. Mean half-life is 7 hours. Evidence of effectiveness: In 14 randomised controlled trials RCTs ; for the treatment of osteoarthritis, etodolac was found to be similar in effectiveness and tolerability to naproxen, piroxicam, diclofenac, nimesulide and nabumetone. In 12 RCTs for the treatment of rheumatoid arthritis etodolac was found to be similar in effectiveness and tolerability to naproxen, diclofenac, piroxicam and sulindac. Major adverse effects: The main adverse effects are gastrointestinal and include dyspepsia, abdominal pain, nausea, and flatulence. In a large trial 7% of patients withdrew prematurely because of GI complaints. RCTs using surrogate markers such as endoscopy scores and GI microbleeding show lower measures with etodolac than with naproxen and indomethacin. A recent systematic review6 has confirmed our previous Therapeutics Letter 4 demonstrating the low risk of serious GI complications in clinical practice with low dose ibuprofen 2400mg day ; . Etodolac and other new NSAIDs were not included in this review; longer clinical experience with these agents are required to determine where they fit in terms of risk. Dose and cost: Osteoarthritis and rheumatoid arthritis: 200-400 mg BID $1.60-$3.20 day ; , see Therapeutics Letter 4 for comparative NSAID prices. Conclusions: Etodolac is a new NSAID which has similar effectiveness to other available NSAIDs. It may prove to have a relatively low incidence of serious GI toxicity, however more data are required.
