Blood samples were drawn at predetermined intervals up to 24 post-dose after either DRF-6196 or linezolid administration. The concentrations of DRF6196 and linezolid in various plasma samples were determined by a HPLC method. Following oral administration maximum concentrations of DRF-6196 were achieved within 0.5 h irrespective of the species. While the doses increased in the ratio of 1: 3: 10, mean Cmax and AUC 0- ; values in mice for DRF-6196 increased in the ratio of 1: 3.87: 8.53 and 1: 2.51: 9.24, respectively. Both the Cmax and AUC 0- ; values increased almost proportional to the dose administered in mice. Following i. v administration, the concentration of DRF-6196 declined in a bi-exponential fashion with terminal elimination half-life of 1.5 h irrespective of the species. The systemic clearance and volume of distribution of DRF-6196 in mice were 1.14 L h kg and 0.66 L kg, respectively after i.v administration, while the respective values in rats were 0.61 L h kg and 0.41L kg, respectively. Elimination half-life ranged between 0.8-1.5 h. Absolute oral bioavailability of DRF-6196 was found to be 80-96% across the test dose range. Methods: Cancer patients or suspected high-risk subjects were addressed to genetic counselling and their familial risk was assessed following the Criteria of Modena. The entire coding region of BRCA1 and BRCA2 was evaluated in 66 probands with family history of breast ovarian cancer on genomic DNA using SSCP and automated sequencing techniques. Results: Preliminary results of molecular analysis showed six pathogenic mutations on BRCA1 gene and five pathogenic mutations on BRCA2 gene. Moreover, a IVS20 + 60 dup12 of BRCA1 gene was detected in two patients. This mutation has been previously associated to an increased risk of familial breast cancer. However, the mechanism by which this intronic mutation would disrupt the function of BRCA1 and its pathogenetic role are still debated. Further studies will be performed to investigate the segregation of this mutation with the disease in the families. In two patients mutational analysis showed a T deletion at nucleotide 18130 of BRCA1 gene, a junction zone of repetitive sequences Alujb and Mer33. Further assays will be employed to evaluate the probable role of repetitive Alu repeats as responsible in alternative splicing or metabolic role. Also in these cases family segregation studies will be performed. The remaining BRCA1 and BRCA2 changes consisted of missense mutations. Conclusions: Results obtained on mutational analysis allowed to detect pathogenetic sequence variants detection rate of 16.6% ; and several unclassified variants. Additional evaluation of the pathogenicity of BRCA1 and BRCA2 sequence variants unclassified variants, intronic mutations and missense sequence variants previously not reported ; may expand our understanding of genetic basis of inherited ovarian-cancer syndrome and help elucidate the prognostic implications of inherited BRCA mutations. BACKGROUND: Hyperimmunoglobulin E hyper-IgE ; syndrome is a rare immunodeficiency characterised by recurrent skin and respiratory tract infections, skeletal and dental abnormalities, chronic eczema, and elevated serum IgE. We describe a family with four hyper-IgE syndrome patients 38, 37, 30 and 7 years old ; , in which we investigated the cytokine response to both specific and non-specific stimulation. METHODS: Whole blood from patients and volunteers was stimulated for either 24 or 48h at 37 degrees C with heat-killed Staphylococcus, C. albicans or a combination of IL-12 and IL-18. Cytokine concentrations in the plasma were measured by specific radioimmuno-assays or ELISA. RESULTS: Serum IgE ranged from 5, 000 to 16, 670 IU ml, and neutrophil chemotaxis was normal in all four patients. Tumour necrosis factor, interleukin IL ; -1beta, IL-6 and IL-8 production after stimulation of whole-blood cultures with lipopolysaccharide or heat-killed S. aureus did not differ between the adult patients and four healthy controls. In contrast, when blood from patients and controls was stimulated with heatkilled S. aureus or C. albicans, a severe imbalance towards a Th2 phenotype was found, with 10- to 30-fold reduction in the IFNgamma IL-10 ratios in the hyper-IgE syndrome patients. The IFNgamma production in the patients was less severely impaired when blood was non-specifically stimulated with a combination of IL-18 and IL-12. CONCLUSION: In this family with hyper-IgE syndrome, the imbalance in the Th1 Th2 cytokine production may have been involved in the pathogenesis of the recurrent infections and or chronic eczema characteristic of this disease. Coincidence of immune-mediated diseases driven by Th1 and Th2 subsets suggests a common aetiology. A population-based study using computerized general practice data. BACKGROUND: The recent rise in the prevalence of immune-mediated diseases has been attributed to environmental factors such as a lack of microbial challenge, or dietary change, that deviate the overall balance between mutually antagonistic subsets of T helper Th ; cells. OBJECTIVE: An alternative proposal is that recent environmental changes have resulted in an immune system that is more likely to produce both Th1 and Th2 responses against benign antigens. The prediction of this hypothesis, that Th1 and Th2-mediated diseases are not mutually exclusive, and may be positively associated, is tested here in a whole population. METHODS: Data from General Practices participating in the Scottish Continuous Morbidity Recording CMR ; project were used to determine the coincidence of the major Th2-mediated atopic diseases; asthma, eczema and allergic rhinitis, with the Th1-mediated autoimmune conditions; type I diabetes, rheumatoid arthritis and psoriasis. We also identified the prescription rates of inhaled therapy for asthma in patients with Th1-mediated disease. RESULTS: There was a significant increase in the risk of presenting with a Th1-mediated autoimmune condition in patients with a history of allergic disease standardized prevalence ratio 95% confidence interval ; 1.28 1.18-1.37 . Likewise, the standardized prevalence ratios of presenting with either eczema 1.67 1.48-1.87 or allergic rhinitis 1.22 1.021.44 were significantly increased in subjects with a history of Th1-mediated disease. There was a particularly strong association between current psoriasis and current eczema standardized prevalence ratio ofpsoriasis in subjects with eczema 2.88, 95% confidence interval CI ; 2.38-3.45 ; . There was also a significant increase in prescriptions for inhaled asthma therapy in patients with Th1 disease. CONCLUSION: It is concluded that Th1- and Th2-mediated diseases are significantly associated in a large General Practice population. This finding supports the proposal that autoimmune and atopic diseases share risk factors that increase the propensity of the immune system to generate both Th1- and Th2-mediated inappropriate responses to non-pathological antigens, for example, maxzide.
6. Specific objective for the coming year: Our successes in recruiting, funding and attracting trainees have resulted in an acute space shortage in the Detwiller Pavilion. Additional space will need to be found to accommodate our greatly expanded research and training programs. This will insure that the Division remains a leading center for Neuroscience research in the future. We also plan to continue to take advantage of new funding opportunities, including Canadian Research Chairs, CIHR, Michael Smith Foundation, etc. to further develop and enhance the Division of Neuroscience. Indeed, a candidate put forward by the Division in the current round of Canada Research Chairs competitions was ranked highest of all the candidates in the Faculty of Medicine. It will be important to ensure that such outstanding applicants have appropriate research space. The members of the Division will continue to work closely with Dr. Max Cynader, the Director of the Brain Research Centre at UBC, to ensure the success of this exciting venture.
